To Schedule an Appointment
We are currently accepting new patients and to serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen.
At your initial visit, you will be asked to present your insurance card, photo ID, a list of current medications, any necessary co-payments, and to complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork ahead of time and bring it with you.

Annual gynecology appointments are mailed approximately 1 month in advance to ensure you have time review your schedule and to call to reschedule if necessary. You may communicate dates and times to the office beforehand to ensure availability.
In order to make your visit run efficiently and to limit the wait time forms will be mailed to you ahead of time. Please complete the patient information worksheet and medication form and bring it to your appointment along with your current insurance card, photo ID, co-pay and any outstanding balance that may have accumulated.
To Cancel or Reschedule
In order to be courteous and accommodate potential emergent care situations, patients are required to contact the office to cancel at least 24 hours prior to their scheduled appointment time. A no show fee will be administered if a cancellation had not been placed.
In Case of Emergency
Live triage staff is able to take calls from 8:30am to 4:30pm Monday through Friday. One of the three Providers is present in the office at these times to direct patient care based on the severity of the situation. One of our three Providers is on call for our patients during evenings and weekends in the event of emergent care needs.
