Insurance plan participation
At Middlesex Obstetrical & Gynecological Associates, we participate in many managed care plans and see patients with government as well as traditional insurances. If we are not your designated gynecologist, some insurance plans may require that you obtain a referral from your primary care doctor before you visit us. To learn if a particular physician participates with your plan, please contact our office or your insurance company.
We participate with the following major plans:
- Anthem Blue Cross and Blue Shield / Wellpoint
- CIGNA Healthcare
- First Health / Coventry Health care
- Harvard Pilgrim Health Care
- Multiplan / PHCS / Emblem Healthcare
- Northeast Health Direct / Consumer Health Network PPO / ChoiceCare Humana
- Oxford Health Plan
- United Healthcare
WHC participates in the following CT Health Insurance Exchange health plans:
- United Healthcare
Note: Given the fact that there are multiple benefit designs being offered and different eligibility requirements, we would recommend that you either go on the AccessHealthCT website at accesshealtct.com or call 1-855-805-HEALTH (1-855-805-4325) for more information and to enroll for health insurance coverage
About insurance and payments
You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or our physicians do not participate with your health plan, full payment also is expected at the time of service.
For your convenience, we accept cash, check, MasterCard, VISA, Discover and AMEX. All returned checks will be assessed a $25.00 service charge. If you are an existing patient with a balance due, you can also pay your bill online.
