Women’s Health Connecticut is currently seeking a Full-time, Licensed Practical Nurse (LPN) at Woodland Women’s Health Associates!
Position: Licensed Practical Nurse (LPN)
Location: Woodland Women’s Health Associates
Address: Float/Travel is required between the Hartford, Avon & Wethersfield offices
Employment: Full-time, 40 hours per week
Schedule: Monday- Friday, 8:30am- 5:00pm
Reports to: Practice Manager
Job Summary: The Licensed Practical Nurse (LPN) is responsible for the delivery of patient care under the direction of the Physician. The LPN prepares and assesses patients for the Providers in-office and triages patient phone calls. The LPN is hands on with providing clinical care including manual vitals, medication review and administration and assists with coordinating and scheduling patient care plans.
Position duties and responsibilities:
- Conduct patient assessments
- Administer prescribed patient’s medication, treatments and therapies according to physician’s orders
- Coordinate care plans
- Triage patient phone calls
- Maintain clear communication with other members of the patients’ multi-disciplinary team
- Fostering a supportive and compassionate environment to care for patients and their families
Skills/qualifications:
- Current state issued LPN license
- Current CPR certification
- Electronic Medical Record experience
- 1 year of LPN experience, OBGYN preferred
- Medical Office/Out-patient practice experience, is required.
- Passion towards providing patient care.
